Citizenship Declaration Forms

Arising from the Irish Nationality and Citizenship Act 2004, the following Declaration forms are required to be completed in certain circumstances by parents/guardians of children born on or after 1 January 2005 only.  These circumstances are set out in the section of this website entitled 'New Citizenship Changes effective from 1 January 2005'
